Requirement of Applicant 
1. Age 18 or above.  (Age under 18, need to be signed and declared by parent/guardian.）
2. A freediver with valid freediving certificate or student in learning or uncomplete. (need submit c-card / course history for proofing. )

Membership yearly fee
HK$250 per annual.
For those who enroll the new freediving course, you can waive this application procedure and 1st year annuel fee. 

Application Method
1. After submitt the below form sucessfully, we will contact you via email. 
2. we will request your freedive c-card or history for proofing 1st. 
 

Important Note
1. Membership valid from 1st Jan to 31st Dec. Membership renewal could be processed from 1st Dec for next year.
2. After submit application form and paid, it means you agree to be bound by the rules, regulations and decisions of HKFDA.
3. It is advisable all students to complete all classes during the membership period. After the membership expired, it will be necessary to pay an additional annual fee for renewal before continuing to attend the remaining classes.
4. HKFDA reserves the right to the final approval of the membership application.

Remarks: Please complete all the procedures and successfully become a paid member before starting any activity registration.
If you are a non-member or have not complete the membership application procedures before registering for any event, all booking will not be accepted and the reservation will be unsuccessful.
